Medical Requirements for New Students

Congratulations on your acceptance to 黑料网!

Required Information

黑料网 Student Health Services reviews your medical information and verifies immunization requirements through the confidential wellness portal. 

黑料网 requires the completion of the following:

  • Medical History

Required immunizations and uploaded official immunization records, copies of blue cards, yellow cards, or state immunization records.

  • (Required: Two doses of MMR, one dose of Meningitis ACWY within the last 5 years, two doses of Varicella, and a Tdap within ten years).

Submission of TB testing (if required) based on your answers to the TB Screening Questionnaire

Students who lack adequate immunizations and/or do not submit immunization information will be unable to check in and receive their room key upon arrival. We strongly recommend you obtain all required vaccines before you arrive at 黑料网. If that is not possible, you can receive your immunizations (at cost) through Student Health Services after your arrival on campus.

Medical or Religious Exemptions

If you are applying for a medical or religious exemption for any immunizations, submit your completed forms online via or email your completed form to studenthealth@colgate.edu.

 

Submit Additional Forms

Some students need to share additional medical information with Student Health Services for continuity of care while at 黑料网, such as:

  • Allergy injection records and orders
  • Medication infusions (Remicade, etc.)
  • Medication injection records and orders (Humira, Somponi, Dupixent, Stelara, Tremfya, Depo-Provera, Xolair)
  • Documentation of any medical conditions for which you will need Student Health Services to provide prescriptions or ongoing care

Additional Information

Student Health Services (SHS) does not have a pharmacy and cannot fill prescriptions written by providers outside the Health Center. We do have a limited dispensary of commonly prescribed medications available for purchase when prescribed by an SHS clinician. If you take a medication that will need to be refilled at school, talk with your medical provider and make a plan to bring enough medication to last the semester or have your physician arrange for refills with one of Hamilton鈥檚 local pharmacies,  and .

  • Bring your insurance or prescription drug card and know your coverage. This is especially important for labs, X-rays, urgent or emergency services, and other specialty services.
  • Know your allergies, medication doses, and health history
  • Emergency contact(s) telephone numbers
  • Bring the contact information for your current health care providers
  • If you have a chronic illness, obtain a letter of explanation from your physician and schedule an introductory appointment with a provider at Student Health Services.
  • If you are under 18, you will need a parental consent form to receive treatment at Student Health Services.

As you prepare for your arrival at 黑料网, we recommend putting together a first-aid kit to have on hand in case of minor illness and injuries.

  • Sore throat medicines/lozenges
  • Sunscreen
  • Tissues
     

 

Wilderness Adventure requires the submission of a separate health history form, distinct from the forms above that is submitted to student health services.
